Presbyopia is directly linked to age, and it develops around age 45, due to a lack of flexibility in the crystalline lens as it ages.
In a normal eye before the development of presbyopia, the shape of the crystalline lens (which is a natural optical lens located behind the iris) changes in shape and power when the eye is changing from near to distance vision and vice versa. This flexibility of the lens is necessary to be able to keep images focused on the retina to keep the images clear.
Presbyopia is not a refractive disorder, because it is due to the gradual loss of focusing power of the crystalline lens, which can no longer focus images on the retina for near vision, so they become increasingly blurry over the years.
Presbyopia affects all eyes, from normal (emmetropic) to those with existing visual disorders, and in this case the visual discomfort of presbyopia is associated with the existing visual disorder.
There is no single solution for the surgical correction of presbyopia. There are different options that depend on each individual case. Surgery is based on either reshaping the cornea with the excimer laser, or replacing the defective crystalline lens.
Reshaping the cornea by laser can create pseudo-accommodation. Another technique is to correct one eye for distance vision and the other for near vision. The crystalline lens may also be removed and replaced with a multifocal implant.
Many factors influence the choice of technique. This depends on the age and profession of the patient, the condition of the eye, and the choices and visual compromises the patient wishes to make
